Each one-page form has blanks for a unique drydock number, date, vessel type, vessel name, draft fore and aft, weight, docking
and undocking details including names of towboats used. There is also room for remarks and boxes at the bottom of the form
for additional numeric information. Occasionally, the reverse of the form is used for notes. Entries are made in pencil.
